Location: REMOTE
Salary: $18.00 USD Hourly - $20.00 USD Hourly
Description: We are currently seeking a bilingual (English & Spanish) Personal Assistance Line Specialist to work remotely. Please see details below.

Schedule: Monday-Friday 8am-5pm PST (Currently only accepting candidates living in the West time zones)

Job Summary: Responds to questions from Dual Eligible Members about their Medicare and Medi-Cal benefits and assists with care coordination, identification and access to needed care and services. The Personal Assistance Line Specialist conducts outbound calls to welcome dual eligible members and receives inbound calls on a dedicated toll-free number.

This job will have the following responsibilities:

• Reviews how the managed care system works- the primary care physician's role, how to get referrals to specialists, medications, etc.
• Facilitates an initial health assessment (IHA) and Staying Healthy Assessment (SHA) appointment with the primary care physician
• Identifies continuity of care issues, such as getting authorization with the new medical group for oxygen, hospital beds, or how to transition the member from a non-formulary drug to a formulary drug.
• Screens the member for case management needs based on pre-established criteria, as well as long-term supports and services needs
• Follows up with medical groups and providers to ensure that members receive their referrals, acts as a liaison between the member, medical groups, and our own case management programs
  

Qualifications & Requirements:

• Bilingual - English and Spanish
• Comfortable working remotely
• Healthcare experience
• Great customer service skills
