Job Summary: The RN Diabetes Specialist provides leadership and clinical expertise across WellStar Health System to enhance care delivery for hospitalized patients with diabetes. This role incorporates evidence-based diabetes management guidelines and standards of care, facilitates change and implements processes to drive glycemic control and achieve organizational quality metrics and performance outcomes. The ideal candidate views the providers and clinicians as customers and actively explores customer needs and collaborates to find creative solutions to satisfy those needs. As a RN Diabetes Specialist, problem solving and clinical management of patients is performed at an expert level of practice. Collaborates with diabetes services team, clinicians and interdisciplinary team members, and system and hospital leadership to achieve diabetes program objectives and enhance clinical outcomes and customer service. Monitors glycemic control, reviews case variances and outcomes and facilitates implementation of strategies to optimize glycemic control (e.g. order set and/or protocol and policy development; ongoing education with hospital staff, and collaboration with hospital interdisciplinary teams to improve care for patients with diabetes. Initiates and/or advises interdisciplinary teams (e.g. quality improvement, patient or medication safety, documentation/tool development, clinical informatics and decision support) as well as actively participates in and integrates current organizational structure, policies and procedures, and forms as related to diabetes care. Identifies, supports, and provides continuing education to hospital clinicians to enhance proficiency in the management and education of patients with diabetes. Works as an integral member of the WellStar team by being familiar with system goals, programs and services. Serves as a resource, support and subject matter expert to system and hospital committees, including guidance regarding certification and/or regulatory requirements. Mentors hospital diabetes education staff ensuring optimal patient assessment, education/intervention, care coordination and documentation to facilitate transitions of care. Provides and/or coordinates orientation and continuing education of hospital diabetes staff. Maintains professional standards for diabetes education and practice that are guided by the American Diabetes Association (ADA) "Standards of Medical Care in Diabetes", Association of Diabetes Care and Education Specialists (ADCES), American Association of Clinical Endocrinology (AACE), and others as appropriate for current clinical practice recommendations and evidence based guidelines. state-of-the-art approach to diabetes care and self-management training. Program Coordination and Staff Development * Mentors hospital diabetes education staff ensuring optimal patient assessment, education/intervention, care coordination, and documentation to facilitate transitions of care. * Provides and/or coordinates orientation and continuing education of hospital diabetes staff. * Serves as a mentor in professional development of diabetes staff. Maintains professional standards for diabetes education and practices that are guided by the ADA "Standards of Medical Care in Diabetes", ADCES, AACE, and others as appropriate for current clinical practice recommendations and evidence-based guidelines Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct. Required Minimum Education:

  • Bachelors Nursing
Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
  • Reg Nurse (Single State) or RN - Multi-state Compact
  • Basic Life Support or BLS - Instructor
Additional License(s) and Certification(s): Certified Diabetic Educator-If not currently certified, applicant must obtain certification within 2 years Preferred Required Minimum Experience: Minimum 3 years progressive experience in a hospital setting as a RN Required or Experience in treatment and education principles with diabetes patients Preferred Required Minimum Skills: Computer knowledge including ability to use Epic, Microsoft Word, Excel and PowerPoint. Excellent organizational, analytical, problem solving and customer management skills. Must have a high attention to detail. Must be able to adapt and flourish in a dynamic environment. Strong written and verbal communication and interpersonal skills. Lean Six Sigma, project management skills, quality improvement skills. Function independently with minimal supervision, as well as work and lead effectively in a team environment. Travel to service locations required via personal automobile. Must be physically capable of initiating CPR.
