Description:
Licensed Practical/Vocational Nurse (LPN/LVN)
Full-Time | Monday - Friday, 9:00 am - 5:00 pm
Job Summary:
We are looking for a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care to patients in their homes. In this role, you will work under the supervision of a Registered Nurse, following physician orders and care plans to ensure quality patient outcomes. This position is an opportunity to provide compassionate care while working in a supportive and collaborative environment.
Key Responsibilities:
- Administer nursing care according to patient care plans.
- Monitor patient reactions to treatments, and medications, and report any changes to the RN and physician.
- Teach patients and families self-care and health practices to support their recovery and well-being.
- Maintain accurate records of patient care, treatments, and observations.
- Participate in patient care meetings and collaborate with other healthcare team members as necessary.
- Ensure adherence to safety protocols and aseptic techniques.
Qualifications:
- Education: Graduate of a state-approved practical or vocational nursing program.
- License: Valid LPN/LVN license in the state of practice.
- Experience: At least 1 year of nursing experience preferred.
- Skills: Ability to work independently, exercise good judgment, and align with the agency's philosophy.
Work Hours and Schedule:
- Hours: Monday - Friday, 9:00 am - 5:00 pm
- On-Call: Nights, weekends, and holidays as needed.
