ABOUT THE JOB

The patient navigator is responsible for supporting our patients in navigating Marathon Health and the customer health benefits as well as support ongoing engagement. The patient navigator will also support our front end and clinical team.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Understand the clients current benefit programs for member assistant
  • Assist member to understand their available employer benefit plan as appropriate.
  • Assist members with communication, enrollment, and paperwork as need for benefit eligibility
  • Verification on members insurance eligibility
  • Engage identified members telephonically, or through internal portal.
  • Collaborate with Clinic Staff when needed to support patients
  • Coordinate referrals, schedule exams, procedures, and appointments for patients
  • Attend Patient Engagement Events
  • Review EMR information and document member-outcomes
  • Performs outgoing phone calls and secure messages to patients: (i) Proactive outreach for lifestyle risk reduction and disease management, including documentation of all interactions in the EMR, (ii) Follow-up on medical visits to the clinic as necessary.
  • Work with membersto identify and removebarriers created by social determinants ofhealth
  • Provide assistance infinding cost affordable options formedications, outsidecare, transportation, groceries, housing, and other Social Determinate of Health issues when needed.
  • Answer incoming calls and general office phones
  • Assist with collection and documentation of pertinent demographic and clinical data
  • Maintain client confidentiality
  • Identify potential referral provider or facilities using various tools and applications.
  • Scanning/ Faxing Documents
  • Provides assistance to secure future appointments or testing.

QUALIFICATIONS

Associate degree in related field and 2+ years' experience with insurance plans, third-party payer practice and guidelines or combination of education and experience. Experience in healthcare preferred.

Pay Range: $25.00-30.00/hr

The actual offer may vary dependent upon geographic location and the candidate's years of experience and/or skill level.
